Digital Submissions Update: The Contractual Change Form and Leavers Forms are no longer paper documents. They must be submitted securely online via the HR Portal. 

To action these, please sign into the HR Portal, select the appropriate form from the navigation, and follow the on-screen steps.
Unpaid Leave Process Change: As of December 2025, manual Unpaid Leave Forms are no longer processed. Unpaid leave must now be recorded directly via HealthRoster.
